Home Visits

Bay Medical Group is proud to provide a dedicated home visiting service for our patients who are housebound or those who live in a care home.

All requests for visits are assessed on the same day we receive them by a clinician, and we ask these are received no later than 4pm but ideally as early as possible in the day to allow us to plan providing appropriate care in a timely fashion. Not all requests for visits will require a visit and may be dealt with over the phone if this is safe.

To request a Home Visit

  1. Download the NHS App
  2. Request an appointment online
  3. Telephone: 01524 235 900

We consider someone to be housebound if they are unable to leave their house (even for shopping), with the exception of hospital appointments using patient transport. There are of course occasions where someone is suddenly unwell and unable to come to an appointment as they usually would, and this is assessed on a case-by-case basis.

Unfortunately, we cannot provide home visits for convenience purposes. In general, it is preferable that our patients are seen in the surgery where we have facilities to make the most thorough and appropriate assessment.

Change or Cancel an Appointment

If you cannot attend your appointment, please let us know.

Missing your appointment without canceling can cause problems for other patients and disrupt the smooth running of our practice. This can lead to:

  • Longer waiting times
  • Waste of resources
  • A risk to others' health
  • and frustration for patients and staff

Ways to cancel

  1. If you have received a text reminder on your mobile phone from Bay Medical Group, and you cannot attend, just reply CANCEL to this text.
  2. Telephone the surgery on 01524 235 900 and press the dedicated cancellation option number 6.
  3. Cancel through the NHS app – Just go to the Health Tab > then click on cancel my GP appointment or reply with the word CANCEL to your appointment text reminder we sent to you.
