Bay (ICC & PCN) introduction
Bay Integrated Care Community (known as Bay ICC) is a team of health & care workers, community, voluntary, faith sector and statutory organisations.

We work together with the shared vision of aiming to improve physical and mental health outcomes, promote wellbeing and reduce inequalities in health including a focus on the wider determinants of health (our social and physical environments). Bay ICC is led by two clinical leads, Dr Gemma Barrow and Amanda Davey who are committed to working in partnership with our communities to help improve population health.
Bay Primary Care Network (known as Bay PCN) builds on existing primary care services and enables greater provision of proactive, personalised, coordinated and more integrated health and social care for people, a shift from sometimes reactively providing appointments to proactively caring for the people and communities they serve. Bay PCN is led by Clinical Director Dr Joanne Price.
In Bay ICC & PCN work together alongside our community, voluntary, health and statutory partners with the aim of improving population health. Meet the team
Within the team there are a number of roles that can support people. These roles are all complimentary and focus on a personalised care approach. We work as a team and if you would like to discuss with us how the roles may be able to support you or a loved one you can contact the team.
